The difference with bioidentical hormones vs. the synthetic alternative is that bioidenticals mimic our body’s own natural hormones in structure and function. In fact, these are naturally derived, as opposed to synthetics, which are derived from equine sources (horses) and are cheap and often have  preservativesless, dye's, fillers and other ingredients we aren't certain of which is why they are covered by insurance.  Furthermore, bioidentical hormones may be customized to a patient's needs and dosing, whereas synthetic hormones are generic dosing.
